So today let NANA's head doctor Hwang answer your most curious questions about transgender breast surgeries!π
π¬Q: How many times have you done breast surgery for transgenders?
Doc. Hwang: Approximately 30 people. Most of the patients are those who are trying to transition to women. At least two people among the patients are those who are trying to become female to male through surgery.
π¬Q: What's the difference from typical breast surgery?
Doc. Hwang: The number of people that have already gone through a complete feminization surgery is very low. Some men still have their male voices, and some didn't inject hormones. Generally, when they come to have surgery from me, the proportion of those who had the hormonal injection and those who hadn't is 50 to 50.
Doc. Hwang: There were no cases of people that have gone through a complete feminization surgery.
π¬Q: How is the procedure process different between those who had hormone injections and those who hadn't?
Doc. Hwang: Those who have received hormone injections, go through the surgery with their glandular tissue more developed and their skin flexible. On the other hand, those who didn't get any hormonal injection barely have any glandular tissue and they have a lot of muscle with tight skin. As they go through surgery under these conditions, the surgery process is a bit more complicated. Due to their muscle mass, the load on my arm will be bigger during the surgery. Another thing is their body size. Usually, males are taller than females, therefore their chest width is wider. So, depending on the chest size, the size of the implant differs.
Doc. Hwang: The wider the chest, the wider the implant, and the capacity of the implant should also be increased. With these requirements met there will be appropriate volume according to the body proportions.
π¬Q: Depending on the muscle mass, does breast surgery become more difficult?
Doc. Hwang: Transgenders usually have a high amount of mass muscle. When I perform a dissection with an endoscope, I make a space for the endoscope to go in with my left hand. If the usual surgery takes 1kg to lift, for transgenders' surgeries, it feels like I'm doing surgery continuously lifting 5kg. That's why when I perform surgeries for transgenders, my left arm aches more.π But that is my problem...π
π¬Q: What type of surgery should be done when women change to men?
Doc. Hwang: When the change is from a woman to a man, they go with breast reduction surgeryπ. Regarding breast reduction surgery, although there are many types of surgery methods, there is a long "O" shaped incision. This is a method where there will be a scar left. With this method, we remove a certain amount of glandular tissue to achieve breast reduction. By removing all the glandular tissue, the patients' chests will become like men's chests.
Doc. Hwang: On the opposite, for men to transition to women, they go through breast augmentation surgery where the scars are hardly visible. But for breast reduction surgeries, the "O" shaped scar leaves patients with some scars.
Doc. Hwang: But if their glandular tissue isn't developed much, then they don't need an "O" type incision. The surgery can be operated just like gynecomastia where we make an incision on the lower part of the areola or it can be removed through liposuction.
